Terms and Conditions

Service Contract

Service will commence following receipt of a signed Service Contract issued to the client company by Occupational Health and Screening Services Limited (OHSS).

Payment

Invoices will be submitted on the last day of each month and will be due for payment within 28 days of invoice date.

All prices are inclusive of shipping costs within the UK. Orders sent outside the UK may be subject to additional shipping charges.

Methods of payment include direct bank transfer or cheque.

Replacement Test Kits

It test kits are lost or damaged by the recipient they can be replaced at a cost of £31 each.  Kits generally have a shelf-life of 6 months.

Test Results & Turnaround Times

Please note that the turnaround time for results is usually within 7 working days. All results will be reported to the client company together with the invoice on the last day of the month, however any abnormal results will be reported to the client company immediately.

Whilst the Exo-foliate Cytology test carries a high rate of accuracy it is not 100% accurate. Occupational Health and Screening Services are not therefore liable in the unlikely event that bladder cancer is not detected.

The turnaround times may be affected by public holidays.

Urine Samples

Any urine samples sent to our laboratory for analysis will be screened only in relation to the possible diagnosis of bladder cancer. All samples tested with a normal result will be discarded/destroyed within 1 month of the test. Any samples tested with abnormal results will be kept for 5 years.

Data Protection and Confidentiality

All medical records are kept in accordance with good practice guidelines.

Insurance

Our insurance cover includes professional indemnity to £1,000,000.

Medical results of all individual’s participating in the corporate urine screening programmes and private urine screenings

To have an accurate record of medical results enabling us to fulfil our service

4. HOW WE PROTECT YOUR PERSONAL DATA
4.1 We will not transfer your personal data outside the EEA without your consent.
4.2 We have implemented generally accepted standards of technology and operational security in order to protect personal data from loss, misuse, or unauthorised alteration or destruction.
4.3 Please note however that where you are transmitting information to us over the internet this can never be guaranteed to be 100% secure.
4.4 We will notify you promptly in the event of any breach of your personal data which might expose you to serious risk.

5. WHO ELSE HAS ACCESS TO THE INFORMATION YOU PROVIDE US?
5.1 We will never sell your personal data. We will not share your personal data with any third parties without your prior consent (which you are free to withhold) except where we are required to do so by law or as set out in the table above or in paragraph 5.2 below.
5.2 We may pass your personal data to third parties who are service providers, agents and subcontractors to us for the purposes of completing tasks and providing services to you. We do this for the purpose of our legitimate interests in operating our business and for performing our services to you. However, we disclose only the personal data that is necessary for the third party to deliver the service and we have a contract in place that requires them to keep your information secure and not to use it for their own purposes unless they are the ultimate service provider. It is possible that third parties may themselves engage others (sub-processors) to process your data. Where this is the case third parties will be required to have contractual arrangements with their sub-processor(s) that ensure your information is kept secure and not used for their own purposes.

6. HOW LONG DO WE KEEP YOUR INFORMATION?
6.1 We are legally obligated to retain your personal data on our systems and in archived storage for up to seven years. Thereafter all personal data will be securely destroyed.
6.2 We securely destroy all financial information once we have used it and no longer need it.

7. INFORMATION COMMISSIONER’S OFFICE
7.1 OHSS is registered with the Information Commissioner’s Office (ICO).

8. YOUR RIGHTS
8.1 You have rights under the GDPR:
(a) to access your personal data
(b) to be provided with information about how your personal data is processed
(c) to have your personal data corrected
(d) to have your personal data erased in certain circumstances
(e) to object to or restrict how your personal data is processed
(f) to have your personal data transferred to yourself or to another business in certain circumstances.
